RALLY Healthcare's primary goal is to provide organisations with access to professional nursing care which can be 'tailor made' to meet their needs. The Guidelines for Referral to RDNS and RALLY Healthcare has been developed with the support of the Department of Human Services, to ensure a standardised and consistent approach in relation to client access to services available from RALLY Healthcare and Royal District Nursing Service. The information below will also assist organisations to access our nursing services.

Establishing a new service
If you have not purchased a service previously from RALLY Healthcare, contact us if you would like to discuss your specific health care needs and how we might be of assistance.

Referring clients to RALLY Healthcare
RALLY Healthcare provides a single point of contact for all referrals. Referrals are received Monday to Friday (excluding public holidays) 7.30am to 5.30pm.

Information required
The following is the minimum client information we require to enable us to decide if we can provide the service you are requesting:
- First and last name
- Date of birth
- Suburb (full address preferred)
- Diagnosis
- Care required for e.g. wound care, post hospital care.
- Timeframe for care (approximate number of visits and preferred visit times).
The following is the minimum client information we require to enable us to accept a referral.
- Financial Authorisation
(applicable for organisations without a Service Agreement - see Financial Authorisation Form - General or Financial Authorisation Form - Aged Care Facilities)
- Client Referral Form
- Medical orders (if applicable)
- Prior to provision of after hours care we require information on: access to property, lighting, animals, household occupants and other safety issues.
Please note: For Hospital in the home referrals, the client's history and consumables required for care are to be supplied to the client and available for the first visit.
Service agreement
To ensure RALLY Healthcare as the provider and you as the purchasing organisation each understand our commitments and responsibilities we prefer to establish a Service Agreement between our organisations. Contact us if you would like to discuss a service agreement to provide services.
