General assessment

A general assessment is provided to clients in their home or during their first visit to a nursing clinic. This includes:

  • Client and carer consultation
  • Prioritising care needs
  • Manual handling assessment to identify issues associated with providing care
  • Identifying issues associated with visiting client after-hours
  • Informing clients of their rights and responsibilities
  • Developing and implementing an initial care plan
  • Referring a client to other appropriate services
  • Providing appropriate information
  • Undertaking re-assessments at appropriate intervals
  • Liaison with other health professionals involved in client's care
