What is it?
The Veterans' Home Care Program provides a range of low level home care services to eligible veterans based on their needs.
The Veterans' Home Care program provides the following services:
- Personal care
- Domestic assistance
- In home respite
- Home and garden maintenance.
- Residential respite
Our nurses can help you by
RALLY Healthcare is funded by the Department of Veterans' Affairs to provide Assessment and Service Coordination for the Veterans Home Care program in Southern and Eastern metropolitan Melbourne and the Mornington Peninsula.
Depending on the needs of the veteran, our Registered Nurses conduct assessments by telephone or, if necessary, in the client's home.
Following assessment, a Service Plan is forwarded to a Veterans' Home Care Service Provider who is contracted by the Department of Veterans' Affairs.
How can veterans access these services?
Access to Veterans' Home Care services is not automatic. Provision of services is based on an assessed need.
Veterans living in Southern or Eastern metropolitan Melbourne or the Mornington Peninsula may contact RALLY Healthcare directly for Assessment and Service Coordination by phoning the DVA national intake number 1300 550 450.
A veteran's doctor or other health professional can refer eligible veterans and war widows/widowers to RALLY Healthcare by completing the VHC General Referral Form and faxing directly to RALLY Healthcare on fax: +61 (3) 9853 4000.
