What is it?
Ahm is an organisation that coordinates the care for members of many private health insurance funds, such as Australian Unity, Australian Health Management and Manchester Unity (see below for full list). The ahm Member Support Program provides a range of support services to eligible health fund clients who are able, and choose to leave hospital earlier than expected, to recover at home.

RALLY Healthcare is one of the contracted service providers for clients receiving nursing services coordinated by ahm and receives referrals for services such as wound care and stomal care.
How can you access these services?
To refer a patient onto the ahm Member Support Program contact ahm on 1800 653 316. ahm will review the requirements, approve funding and arrange for RALLY Healthcare or another contracted home nursing provider to provide care to the client.
ahm continues to monitor the ongoing needs of the client in conjunction with the Service Provider and Medical Practitioner as required.
Clients are discharged from Hospital and do not retain in-patient status. Ongoing medical care is managed and approved by the client's general practitioner and medical specialist follow-up.
